employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
Employees were accessing the mezzanine that was 7 feet high to retrieve
merchandise
with a portable stair system that was 5 feet high.  Employees would climb
the handrails
of the portable stair system to access the mezzanine, which exposed them
to fall hazards.
