Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards while riding on the rear area of a large vehicle that was backing up: (a)GTM, LTD & 915 4th Street, Meeker, CO: On or before March 24, 2005, the employer did not ensure that employees were adequately trained to safely operate or work on/around a large trash truck while the vehicle was backing up. An employee was struck by a Volvo GM Heavy Truck with a PackMaster rear-load trash compactor. The employee at the rear of the truck was struck by and run over as the truck traveled approximately 35 feet while backing into a cul-de-sac to pick up residential trash. The employees followed a practice of riding on a rear side step on either side of the truck or on the back lip of the trash compactor. These practices were contrary to safety procedures in the PackMaster Operator's Manual and safety procedures on warning stickers located at the rear side steps of the PackMaster compactor. Among other methods, one feasible and acceptable abatement method to correct is to: (a)Provide training to each employee, prior to operation of trash truck, and monitoring compliance specifically related to safety procedures listed on pages 2-6 (items 3, 4, 5, 20 & 21) and 3-6 of the PackMaster Operator's Manual and on decals as noted 2-3 and 2-5 (items 30 & 43) of the PackMaster Operator's Manual. Abatement Note; Abatement certification and documentation are required for this item.

Section 5(a)(1) of the Occupation Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards while working on/around a large vehicle that was backing up without a functioning backup alarm or effective alternative: (a)GTM, LTD & 915 4th Street, Meeker, CO: On or before March 24, 2005, the employer did not ensure that employees working on or around a large trash truck were warned when the truck was put in reverse and traveling backwards. An employee at the rear of a trash truck was struck by and run over by the rear wheels while the truck was backing up. The installed back up alarm did not work and an effective procedure for a spotter or other alternative was not available. This was contrary to safety procedures in the PackMaster Operator's manual. Among other methods, one feasible and acceptable abatement method to correct is to: (a)Repair the backup alarm before operating the truck or provide an effective alternative specifically related to safety procedures listed on pages 2-6 (item 24), and 3-9 of the PackMaster Operator's Manual. Abatement Note: Abatement certification and documentation are required for this item.
