General-duty citation text

Section 5 (a) (1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falling:
(a)Workplace/Garage - Employees attempted to move an out of service Hydro
Traveler (water cannon) weighing more than 2,000 lbs. while steering one
end
of the load with an OTC Stinger Series 3 Ton hydraulic service jack as the
other
end of the load was pushed by use of a skid steer loader.
A steel beam approximately 34 inches long by 4 inches wide was placed
between
the jack and the load when it was raised from a static position.
Employees then
tried to steer one end of the load by manipulating the jack handle and
caster
wheel assembly.  While the load was being moved horizontally it fell
toward the
employee;  On or about 06/28/05.
ABATEMENT NOTES:
Among other methods, one feasible method to reduce this hazard it to:
1.Follow the manufacturer's Safety Precautions which states in part;
a."Use the jack for lifting purposes only".
b."Do not use blocks or other extenders between the
saddle and the load being lifted."
c."Read, understand, and follow all instructions,
including ASME [American Society of Mechanical
Engineers] PALD [Portable Automotive Lifting
Devices];  Part 10 for service jacks;
d.If the operator cannot read English, operating
instructions and safety precautions must be read and
discussed in the operator's native language."
2.Follow ASME PALD Part 10;  Section 3 Product Marking;  Para.
3.1 Sample Warning;  as follows:
"WARNING:  THIS IS A LIFTING DEVICE ONLY.  DO NOT
MOVE OR DOLLY THE VEHICLE WHILE ON THE JACK.
IMMEDIATELY AFTER LIFTING SUPPORT THE VEHICLE
WITH APPROPRIATE MEANS, STUDY, UNDERSTAND,
AND FOLLOW ALL INSTRUCTIONS.  FAILURE TO HEED
THIS WARNING MAY RESULT IN PERSONAL INJURY
AND/OR PROPERTY DAMAGE."
3.Use the appropriate hoisting equipment rated for the load carried
when lifting.
4.Use an appropriate heavy duty dolly or spreader cart when
transporting.
ABATEMENT DOCUMENTATION IS REQUIRED AS OUTLINED IN PART
29 CFR 1903.19
