5(a)(1)

Serious Gravity 10 1 instance 1 exposed
Issued
Abate by
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which was free from
recognized hazards
which were likely to cause death or serious physical harm to employees, in
that, employees
were exposed to crushing and tipping injuries due to inadequately
inspected cranes:
a)Employees were exposed to serious injury when working on a HIAB 210K
articulating boom crane that had not been adequately inspected or
serviced. A bolt that held
the handhold for accessing the work seat had vibrated loose, a movable
step on the back of
the truck had been removed, and slip proofing material that had been
placed on the stairs was
bubbled up and detaching from the stairs.
Among other methods, one feasible and acceptable abatement method to
correct this
hazardous condition would be to follow the inspection criteria found in
the Warranty and
Service Manual for the HIAB Cargotec Crane or the HIAB Planned Maintenance
Crane
Inspection Report as well as the criteria set forth in ASME B30.22-2000,
Articulating Boom
Cranes, in sections 22-2.1.3 Frequent Inspection and 22-2.1.4 Periodic
Inspection.
