Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of being struck by or crushed by loads.
Operators that are not
trained to ASME B30.2 1990 Section 2-3.1.4 "Qualifications for Operators
of Floor Operated
Cranes" in the proper use of lifting/rigging equipment, methods, and
cribbing to move and stack
bundles; and stand beside and between unsecured stacks of bundled steel
box channel while
materials are being moved:
a)  On or about 5/26/04, in the stock holding area, while operating the
Otis 10 ton
Overhead Crane, SN 12340, an employee standing in between stacks of
unsecure bundles
of steel, was placing (3) steel bundles on top of another bundle of steel.
The bundles
collapsed pinning the employee's legs under the falling steel bundles and
collapsing
(2)
additional stacks of bundled steel behind the employee. One other employee
and one
supervisor were exposed to the secondary collapse.
Among others, a feasable method to eliminate or reduce this hazard; (a) is
to comply with
AMSE Standard B30.2 Section 2-3.1.4, "Qualifications for Operators of
Overhead Cranes"; (b)
establish a program in the proper placement and use of wood cribbing,
lifting/rigging equipment,
and procedures to move and stack bundles, then train employees in these
procedures; (c)
establish material handling procedures that prevent unsecure stacking and
moving loads that put
the employee between stacks/bundles, then train the employees in these
procedures.
