Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by: The employees were exposed to a struck-by hazard due to the improper installing of the rotary bushings on rig floor, the bushings were not secured to prevent them from coming out and off the rotary table. The employer failed to install the locks required. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install the rotary bushings with the required locks to prevent it from coming apart.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard: Employees were exposed to a struck-by hazard due to improper installation of Crosby-clips on wire rope on the mud bucket. The employer improperly installed the wire rope clips and failed to install thimbles at the ends of the wire rope used an eye to lift the mud bucket. Among other methods, one (1) feasible and acceptable method to correct this hazard is to install the Crosby-clips properly. Thimbles are recommended to be used instead making an eye from the wire rope.
