employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by hazards:
On August 3, 2004 and times prior thereto, an employee was exposed to being
struck by a overhead front loader bucket while laying on top of a grain fan
performing maintenance.  The loader was being used to lift the grain fan
by using
rope to pull the fan back into its proper positioning.
