At this rig site, the employees were exposed to a struck-by hazard due to
standpipe not properly secured with adequate bolts on the clamps attached
to the
frame and rig floor.
a.Standpipe on west side of the rig floor going to the ground level was not
properly secured and rattled during drilling.
b.Standpipe secured to rig floor had a broken bolt head.
c.Standpipe secured to the mast had a bolt missing.
Among other methods, one (1) feasible and acceptable method to correct this
hazard is to secure the standpipe with the required bolts as specified in
the
Accident Prevention Guide, Recommended Practice for Occupational Safety for
Oil and Gas Well Drilling and Serviceing Operations and Recommended Safe
Procedures and Guidelines for Oil and Gas Well Servicing.
