Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards related to failure and/or inoperability of
major systems including,
but not limited to, the hydraulic systems, bucket controls, mechanical
components, etc., on the
Versalift aerial lift(s).
a) Telford, PA worsite - An employee performing work from a Versalift
V-28-PS aerial
lift mounted on a 1973 Ford truck fell to the ground when the lift failed,
on or about
december 7, 2005.
Among others, one useful and feasible abatement method would be to have
all such equipment
inspected and tested by a competent mechanic, in accordance with
manufacturer's Schedule and
Record contained in Versalift Owner's Manual for the Model V-28 aerial
lift, to ensure that the
mechanical and structural systems on the Veralift(s) are in sound and
properly functioning
condition.  Furthermore, aerial lifts where defects must be propmptly
removed from service.
Abatement certification required within 10 days after abatement date.  The
certification shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.
