SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employee operating forklift was exposed to the hazard of falling from or being pinned or crushed underneath forklift. A feasible and useful method to correct this hazard, among other methods, is to install and enforce use of a seat belt on Hyster forklift, Model Number 40, Serial Number C108V04178, located in the establishment. B. Employer knew or should have known that employee operating forklift was exposed to the hazard of falling from or being pinned or crushed underneath forklift. A feasible and useful method to correct this hazard, among other methods, is to install and require use of seat belt on Catapillar forklift, Machine #6, 50510, Model Number M30, Serial Number 99N828, located in roller buffer area.

SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employees working behind truck were exposed to the hazards of being crushed by truck back up without warning. A feasible and useful method to correct t his hazard, among other methods, is to install a back up alarm on Ford flat bed truck, no model or serial number, located at Dock 5 at establishment.
