SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employees operating forklift were exposed to the hazard of falling from or being pinned or crushed underneath forklift. A feasible and useful method to correct this hazard, among other methods, is to install seat belt on TCM forklift #5, Model Number FG15, located in warehouse at establishment. B. Employer knew or should have known that employees operating forklift were exposed to the hazard of falling from or being pinned or crushed underneath forklift. A feasible and useful method to correct this hazard, among other methods, is to install seat belt on Nissan 20 forklift, #7, no model number, no serial number, located in warehouse at establishment. C. Employer knew or should have known that employees working behind truck were exposed to the hazards of truck backing up without warning. A feasible and useful method to correct this hazard, among other methods, is to install back-up alarm on Chevy truck, Tag Number P200583, located in yard at establishment. D. Employer knew or should have known that employees working behind truck were exposed to the hazard of truck backing up without warning. A feasible and useful method to correct this hazard, among other methods, is to install back-up alarm on Ford truck, Tag Number P200584, located in yard at establishment.
