Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized
hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to moving conveyor belt hazards:
a)  On or about February 7, 2006 and at times prior, Northwest Concrete
Product dba
Northwest Logistics did not ensure that employees were protected from
moving conveyor
belt hazards.  The operator was carried by the belt through the wheel
tensioner area and
up into the covered conveyor system.
Among others, some feasible and acceptable methods of abatement to correct
this hazard are:
a)  Installation of adjustable guardrails along the conveyor belt.
b)  Installation of emergency stop cable along points of exposure.
c)  Installation of grating over exposed conveyor belt areas.
Abatement Note:  Abatement certification and documentation is required for
this item (see
enclosed "Sample Abatement-Certification Letter").
