employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to flying plastic projectiles hazards due to mechanical
failure of the facility air
system operating at a pressure in excess of 100 psi as referenced in
ANSI/ASME B31.8 1999.
a)On and prior to July 5, 2006, a one inch PVC pipe air system was used to
transport compressed air from the Ingersoll Rand air compressor in the
Back Bay
area, north side of the building, with an operating pressure of 125 psi.
Among other methods, one feasible and accepted means to correct this
hazard is
to replace the current PVC compressed air piping system with materials
suitable
for maximum air pressure such as metal pipes or other approved hoses.

were exposed to suspended loads held by overhead hoists that were not
periodically inspected:
a)Hitch area, south end;  Employees were exposed to the hazard of being
struck by
falling loads, such as sections of metal, by using two Coffing 1 ton
underslung
cranes that had not been inspected with a periodical inspection (at least
every 12
months) to determine wear, deterioration and malfunction as per ANSI
B30.16-1.
Both cranes had the safety latches missing from the load hooks.
One method of abatement, among others, would be to have this electric
hoist
inspected on a
periodic bases (at least every twelve months) as required by ANSI B30.16-1
and then to make
necessary repairs.
