Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: a) McKean County, Norwich Township, OFG 35 Well - On or about November 21, 2006 at a remote gas well site employees were exposed to a struck-by hazard when the flow line was not secured at the discharge end and rotated during a blow down procedure. ABATEMENT NOTE: Among other feasible and acceptable abatement methods to correct this hazard: 1. Secure the flow line at the discharge end and well head assembly to prevent rotation. (Reference: American Petroleum Institute (API) Recommended Practice (RP) 54-1999, Paragraph 12.4.3) 2. The completion of a Job Safety Analysis (JSA) prior to employees taking actions as recommended in the International Association of Drilling Contractors (IADC) Accident Prevention Reference Guide.nce
