were exposed to the hazard of being struck by the load should the overhead
hoist or below-the-
hook lifting device fail:
a)Rolling Mill, assembly table, on or about 5/31/06:  A below-the-hook
lifting
device used for lifting pallets had not been load tested, had no rated load
capacity markings on the device and no inspections had been conducted on
the device.
Among other methods, several feasible and acceptable abatement methods to
correct these hazards are to:
1.Conduct a rated load test on the device in accordance with ASME
B30.20, Section 20-1.4.
2.Mark the rated load of the device on the main structure or equivalent
in accordance with ASME B30.20, Section 20-1.2.
3.Conduct frequent and periodic inspections of the device in accordance
with ASME B30.20, Section 20-1.3.
b)Rolling Mill, assembly table, on or about 5/31/06:  There were no
inspections
conducted on a Dayton 500 lb. overhead hoist.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to:
1.Conduct frequent and periodic inspection of the overhead hoist in
accordance with ANSI B30.16-1993, Section 16-2.1.
Abatement certification must be submitted for these items.
