Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls due to ladder misuse:
a.Service area - On 03/07/06 employees were required to access an elevated
storage
area via a step ladder.  The size of the ladder and the configuration of
the space
was such that employees were required to crawl over the top of the ladder
to
remove items from the rear of the space.  This was not in accordance with
the
manufacturer's specifications for use of the ladder and placed a lateral
force on
the ladder that exposed employees to the hazard of falling approximately 8
ft.
onto a concrete floor if the ladder tipped.
ABATEMENT NOTE:
Follow the manufacturer's instructions for safe use of the equipment.
Select the appropriate equipment for the job.
Do not store materials in areas that can not be easily and safely accessed.
