Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from metal plate connected wood roof trusses that were subject to potential movement due to lack of proper temporary bracing and proper handling: a. On or about February 2, 2006, at the 5012 West Piute Drive jobsite, Beverly Hills, FL., during the installation of metal plate connected wood roof trusses spanning 44 feet 3 1/4 inches, a truss that had just been set came apart at a splice plate, fatally injuring an employee who had been working on the top of the flat topped truss without fall protection, approximately 16 feet above the concrete floor. The following factors contributed, or may have contributed to the incident: 1. Top Chord Short Piece Temporary Lateral Bracing on the east and west sides wereof 1-inch by 4-inch construction, rather than 2-inch by 4-inch as specified in step 4 of BCSI-B2. 2. Bottom Chord Temporary Lateral Bracing consisted of furring strips rather than 2- inch by 4-inch lumber as specified in step 7 of BCSI-B2. 3. Top and Bottom Chord Temporary Diagonal Bracing was non-existent for this house as specified in steps 5 and 8 of BCSI-B2. 4. Trusses were released from the hoisting supports before all temporary bracing was installed as specified in step 4 of BCSI-B2. 5. Trusses exceeding 30 feet were hoisted using a two-point method rather than a properly configured spreader bar as specified in BCSI-B1. 6. Bracing connections were made with a single 8 penny nail instead of either 10,12 or 16 penny nails as specified in step 4 of BCSI-B2. 7. There was an interior bearing wall that the building plans showed should have been in place that was missing. The truss was designed for three bearing points instead of the two that were provided. One feasible and acceptable method of abatement is to install and handle the roof trusses in accordance with Building Component Safety Information sheets provided by the Truss Plate Institute and the Wood Truss Council of America. The manufacturer provided a copy of the BCSI-B1 summary sheet when the trusses were delivered. the Standard Building code refers to the BCSI for handling, installing and bracing metal plate connected wood roof trusses. Also, install the bearing wall or other appropriate bearing support as required and approved by the building plans and designed by an appropriate engineer or architect.e
