North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish to each of his employees conditions of employment
and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to
employees:
a)Facility:  The employer failed to perform a rated load test for each of
the eight [8] hoist systems
containing bottom-running hoists installed within the facility.
REFERENCE:ANSI/ASME 30.11 - Monorails and Underhung Cranes:
Chapter 11-2 - Inspection, Testing, and Maintenance:
Section 11-2.2.2 - Load Test:
a)New, reinstalled, altered, repaired, and modified equipment should be
load tested prior to initial use as determined by a qualified person.
NOTE:Among other methods, one feasible and acceptable method to correct
this hazard is to
have a rated load test of each of the hoists/hoist systems installed at
the facility
performed by a competent person and to maintain the documentation of the
load
tests on
site in a permanent file for the operational life of the hoist systems.nal

a)Facility:  The employer did not establish an effective inspection
program to ensure frequent and
periodic inspections by a competent person of the eight bottom-running
hoists installed within
the facility.

Chapter 16-2 - Inspection, Testing, and Maintenance:
Section 16-2.1.1 - Inspection Classification:
b)1)Frequent Inspection:  Visual examinations by the operator or
other designated person with records not required:
a)Normal Service:  Monthly.
b)Heavy Service:  Weekly to monthly.
c)Severe Service:  Daily to weekly.
c)2)Periodic Inspection:  Visual inspection by a designated person
who makes records of external conditions to provide the basis
for a continuing evaluation:
a)Normal Service:  Yearly.
b)Heavy Service:  Semi-annually.
c)Severe Service:  Quarterly.
NOTE:Among other methods, one feasible and acceptable method to correct
this hazard is to
develop and implement an inspection procedure for the hoist systems/hoists
present
within the facility that ensures the required inspections covering the
items listed in
B30.16 - Chapter 2 and/or recommended by the hoists' manufacturers are
performed by
competent personnel in compliance with the schedules contained in the
Chapter and to
maintain documentation of these inspections on-site in a permanent file.

supporting the 3/4-ton Dayton
chain hoist was not marked on the rail such that the marking was clearly
legible from the floor.
b)Facility - - Receiving/Shipping Dock:  The rated load capacity of the
monorail supporting the
yellow, unlabeled manual-pull chain hoist was not marked on the rail such
that the marking was
clearly legible from the floor.
c)Facility - - Receiving/Shipping Dock:  The rated load capacity of the
yellow, unlabeled manual-pull chain hoist was not marked on the hoist or
the hoist block such that the marking was clearly
legible from the floor.

Section11.1.1 - Markings:
11-1.1.1 Rated Load Markings - Crane:  The rated load of the crane shall be
marked on the crane, and if the crane has more than one hoisting unit, each
hoist shall have its rated load marked on the hoist or its load block so
that the
rated load marking shall be legible from the ground or floor.
11-1.1.2 Rated Load Markings - Monorails:  The rated load of each hoist on
a
monorail shall be marked in accordance with ASME B30.16 - Overhead Hoists
(Underhung).
"ASME B30.16-1.1.1 Rated Load:  The rated load of the hoist shall be
marked on the hoist or its load block and shall be legible from the
ground or floor."
NOTE:Among other methods, one feasible and acceptable method to correct
this hazard is to
clearly mark the rated load capacity of the crane/monorail on the rail
such that the
marking is legible from the floor and to clearly mark the rated load
capacity of each
hoist on the hoist or the hoist's load block such that the marking is
clearly legible from
the floor.e
