Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment, and a place of employment which were free of recognized hazards that were causing, or likely to cause death or serious physical harm to employees in that: Employee(s) was exposed to fall hazards while performing job related duties when employee(s) was permitted to work from a 4x8 sheet of plywood lifted by the forks of a Terex TH844C forklift. The Terex TH844C forklift should not be used to elevate personnel, as it is not designed for this purpose. One feasible, and acceptable abatement method among others to correct this hazard is to comply with the operators manual, and placard at operators controls that read: Never equip machine with any form of personnel platform or allow personnel to ride on attachment.
