employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fire or explosion hazard due to no Geronimo Emergency
Escape Device in
place before employees began tripping pipe:
On September 21, 2006, and at times prior thereto, the employer failed to
ensure
employees working from elevated platform had the Geronimo Emergency Escape
Device in place before beginning work.
a.The Geronimo Emergency Escape Device Line was not properly installed
next to the monkey board 55 feet above ground. The line dropped down
the southwest derrick leg and was not attached at the terminal end and the
derrickman had no safe means of egress.
b.The Emergency Escape Device Line was not anchored/staked out as
recommended by the manufacturer Geronimo Manufacturing, Inc.
Among other methods for abatement, one feasible and acceptable means to
correct
this hazard would be the installation of a properly rigged and secured
emergency
descent device or other satisfactory auxiliary means of egress, such as
specified
in Section 5 sub-section 5.7, Paragraph 5.7.1 of American Petroleum
Institute
(API) RP 54-92, page 12.
