Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which are free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in the employees
were exposed to:
On or about November 21, 2007, employees were exposed to a struck by
hazard from a
threaded pipe under pressure.  The employees were breaking out the
equipment to a tank
battery.  The discharge line of a compressor was not bled off prior to
working on the line.
The line sheared off striking three employee(s).  The pressure on the
casing was
approximately 1150 psi.  The wellhead, HES #1, is located on CR 401 in
Alvarado, Texas.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to ensure that there is not any residual pressure in the line by
bleeding off the line prior to
disassembling it.
