Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flash fires and explosions: a) On or about January 11, 2007 and at times prior, Pioneer Natural Resources Co. did not ensure that employees and workers were protected from flash fire and explosion hazards. 1. Gas sensors placed on each compressor unit were not installed completely. The sensors were not connected to a warning alarm or monitor system. 2. Fire detection devices (flash eyes) were not installed completely. The devices were not wired to warning alarms, emergency shutdown systems, or monitor systems. 3. Emergency shutdown controls were not installed completely. The controls were not wired into emergency shutdown systems. One feasible and acceptable method of abatement would be to complete the installation of the fire detection and prevention systems which were partially installed at the site.
