#440, did not furnish employment and a
place of employment which were free from recognized hazards that were
causing or likely to cause death or
serious physical harm to employee(s) in that:
(a) At 145 North Loudoun Street, Winchester, VA. the employer had
employee(s) exposed to an inclined
roller conveyor system that was elevated to approximately 16.5 inches
above horizontal where the effect of
gravity caused merchandise cartons to move down the conveyor. The roller
conveyor was not equipped with an
antirunaway, brake or backstop devices that would protect employees form
uncontrolled lowering of loads as
referenced in ASME B20.1-1990 Section 5.5 - Backstops and Brakes.
One feasible and acceptable abatement method would be to design, fabricate
and install a braking device and
backstop to reduce the foreseeable risk of accidental struck by hazards
caused by cartons moving down the
conveyer at speed.
