Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to fall hazards:
On or about June 2, 2007 the employer did not ensure that the platform
used by employees to
perform work near the storage racks was properly secured to the powered
industrial truck.  The
work platform was secured to the forks using a miller lanyard, tied in a
knot.  Employee was
exposed to a fall hazard of approximately 32 feet to the concrete floor
below.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: Secure work platform to the lifting carriage of/or
forks in
accordance with ASME B56.1-2000.
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement
of the
above violation is completed.
