Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: (a)300 Second Street, Harrisburg, PA - An employee (window washer) using a manual Fisk descender and counterweighted outrigger suspension device was not tied off properly in that the support or lifeline and working or descent line were tied off to the same outrigger device. Also, the Roof Top Roller outrigger suspension device was not anchored to a secure anchorage point with a wire rope equivalent in strength to the suspension rope but in no case less than 5/16 in diameter, thereby exposing the employee to a fall hazard of approximately one hundred and twenty (120) feet to the ground below, on or about June 3, 2008. Among other methods, feasible and acceptable methods to correct this hazard is to comply with equipment manufacturer's instructions and/or ANSI/IWCA (International Window Cleaning Association) Standard I - 14.1 (2001). This includes but is not limited to Section 5.7 for Rope Descent Systems, including section 5.7.17 (f) for horizontal movement of a worker suspended from a transportable device, Section 9 for Anchorages and Fall Protection, Section 14 on Rope Descent Systems, including prohibiting the use of descent devices where the manufacturer specifically does not allow their equipment to be used for window cleaning activities, and Section 17.5 for Counterweighted Outriggers. VERIFICATION REQUIRED
