North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)Facility - - Plant #2 - - Dye House: Employer failed to perform a rated load test of the hoist system installed at the extractor / dryers when the system was modified by welding an extension of the support beam for the hoist. REFERENCE:ANSI B30.11 - Monorails and Underhung Cranes: Chapter 11-1 - Construction and Installation: Section 11-1.3.5 Modifications: A rerated system of one whose load-supporting components have been modified shall be tested in accordance with Section 11-2.2. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to have a rated load test of this hoist system performed by a competent person in accordance with the requirements of ANSI B30-11, Section 11-2.2.2.nts

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)Facility: Employer failed to perform frequent and periodic inspections on the underhung hoists operated within Plants #1 and #2. REFERENCE:ANSI B30.11 - Monorails and Underhung Cranes: Chapter 11-2 - Inspection, Testing, and Maintenance: Section 11-2.1.2 - Frequent Inspection: (a)Frequent inspections shall be performed at intervals defined in paragraph 11-2.1(b)(1) and shall include observations during operation. Section 11-2.1.3 - Periodic Inspection: (a)Periodic inspections shall be performed at intervals defined in paragraph 11-2.1(b)(2). NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to implement a program of frequent and periodic inspections of all underhung hoists by a competent person that meet the requirements of ANSI B30.11, Section 11-2.1.2, and Section 11-2.1.3.
