Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were not provided with a platform which met the requirement in the
American Society of
Mechanical Engineers an American National Standard (ASME/ANSI B56.1-1988)
Safety
Standard for Low Lift and High Lift Trucks for elevating personnel:
Date: On or about Tuesday June 17, 2008
Location: Rear of the building near the maintenance shop area at 413
Industrial Blvd
McDonough, GA 30253.
Equipment: Toyota Forklift Truck (Model - 42-6FGU25/Serial # 63029)
Condition: Two employees were observed being elevated to the roof of the
facility at heights up
to 14 feet with a forklift with the platform that was not secured safely
to the forklift and did not
protect the employees from potential fall hazards.
Feasible and Useful Method to correct the Hazard - A feasible method to
prevent this hazard
from occurring again is to provide the employees as safe means to access
the building and/or
equipping the forklift with a platform that meets the ASME/ANSI B56.1-1988
Safety Standard
for Low Lift and High Lift Trucks para 4.17.1 (a-f).
No abatement certification or documentation is required for this item.
