(a) Facility, 685 State Street, Vanport, PA, 15009:
On or about 1/30/2008, employees used and worked in the vicinity of sheet
lifters, a type of
below the hook lifting device, and the sheet lifters were not marked with
their load rating, had
not been subject to frequent and periodic inspections, and the operators
were not trained in the
safe use of the sheet lifters.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable means of
abatement
to correct this hazard is to:
1.Have the sheet lifters load tested and mark them according.
2.Conduct frequent and periodic inspections on the sheet lifters.
3.Train the operators in the safe operation of the sheet lifters.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that heavy loads
are suspended from jib hoists and underhung cranes for which frequent
inspections have not been
conducted, resulting in the operation of the hoists and cranes with
defective parts affecting the
safe operation of the hoists and cranes.
(a) Facility:  On or about 1/30/08, employees used and worked in the
vicinity of approximately
13 jib hoists and two "underhung" cranes which had not had frequent
(monthly)
inspections since
2005.
(b) Weld Shop:  On or about 1/30/08, the free standing two ton Apollo jib
did not have a trolley
stop on the mast end of the jib boom.
(c) Weld Shop: On or about 1/30/08 the jib by Column 2, (CM hoist, serial
# L-241B1) had a
stretched hoist chain with worn links.
(d) Weld Shop: On or about 1/30/08 the jib by Column 5, (CM hoist, serial
# L-70915V) did
not have trolley stops on the mast end of the boom to prevent the trolley
from hitting it.
(e) Weld Shop: On or about 1/30/08 the jib by Column 8 (CM hoist, serial #
L-14CF4) did not
have functioning limit switch.  In addition, there was no emergency stop
button on the push
button station.  The up direction at times sticks and a strain relief
system is needed.  Stops on
the mast end of the boom need to be installed to prevent the trolley from
hitting
it.  The dead
end of the hoist chain link is broken from the housing bolt.
(f) Weld Shop:  On or about 1/30/08, the jib by Column 10 (CM hoist,
serial # L-140F2)
requires a new grommet on the power cable to the power cabinet connection.
There were no
trolley stops to prevent the hoist from hitting the wall.  The tie rod end
for the jib was bent.
Also, there was weld spatter on the hoist chain.
(g) Weld Shop:  On or about 1/30/08 the jib by the Burn Table had the
bottom broken off of
the push button station.  There were no stops on the mast end of the boom
to prevent the trolley
from hitting it.  Also, the lower links of the hoist chain have burn marks
on them.
(h) Machine Shop:  On or about 1/30/08 the jib by Column C (#7, CM hoist,
serial #L-5364Y)
did not have rotation stops that would prevent the jib boom from hitting
the adjacent ladder.
Also, there are no stops to prevent the trolley from hitting the jib mast
and
damaging the hoist
control cover.
(i) Machine Shop:  On or about 1/30/08, the jib by Column E (#9, Yale
hoist, serial #W51083)
has a hoist that is drifting. One holding brake lining is loose on the
shoe.  The jib boom can hit
adjacent equipment.  There are no rotation stops for the jib boom.  The
main line disconnect
switch is not independent: it is on a heater circuit.  There is no
emergency stop button on thepush button station.  The jib boom drifts to
the one side.  The control panel is cracked at the
limit switch connection.
(j) Machine Shop: On or about 1/30/08, the jib by Column B (#10, CM hoist,
serial # L-129CX)
does not have an emergency stop button on the pendant station.  The hoist
chain is twisted and
stretched.  In addition, an incorrect nut and bolt are being used to hold
the chain to the hoist.
(k) Weld Shop:  On or about 1/30/08, the jib by Column F (#13, CM hoist,
serial # L-3516R)
does not have jib boom rotation stops.  The jib boom can hit adjacent
equipment.
(l) Machine Shop:  On or about 1/30/08, the jib by Column D (#20, CM
hoist, serial # L-
33BFO) does not have trolley stops.  The trolley stops by hitting the
column.  Also, the one ton
hoist is on a jib that is marked 2 tons and not marked to match the hoist.
(m) Machine Shop:  On or about 1/30/08, the jib by Column E (#30, CM
hoist, serial # 1086C)
does not have an emergency stop button on the hoist push button station.
The hoist does not
have an independent main line disconnect switch.  There are no rotation
stops to prevent the jib
boom from hitting an adjacent fan.  Also, there are no trolley stops on
the mast end of the
boom.  In addition, the hook is attached to hoist chain by using an
unapproved bolt.
(n) Machine Shop, Allegheny Plastics Area:  On or about 1/30/08, the free
standing jib (#21,
CM hoist serial # L0041MN) does not have an emergency stop button on the
push
button station.
The jib nor the hoist does not have an independent main line disconnect
switch.  There are no
rotation stops to prevent the jib boom from hitting an adjacent fan.
There are no stops on the
mast end of the boom to prevent the trolley from hitting it.  Also, the
hook is attached to the
hoist chain using an unapproved bolt.
(o) Allegheny Plastics Department:  On or about 1/30/08, the #4, 5 ton
underhung crane needs
to have the main tag line festoon replaced.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable means of
abatement
to correct this hazard is to conduct frequent and periodic inspections on
all of the jib hoists  in
accordance with ASME B30.11-2004, "Monorails and Underhung Cranes", and
correct all
deficiencies to ensure the proper and safe operation of all jib hoists in
the facility.

the operator restraining device (seat belt) supplied/installed in the
truck.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable means of
abatement
to correct this hazard is to:
1.Train and/or retrain employees in accordance with ANSI/ITSDF,
B56.1-2005, Safety
Standards for Low Lift and High Lift Powered Industrial Trucks.
2.Ensure that employees who are operating powered industrial trucks use
the personal
restraining devices supplied and installed by the equipment
