On or about February 17, 2026, the employer failed to establish and implement a written respiratory protection program with worksite-specific procedures for employees required to use RPB Safety Nova 3 powered air purifying respirators during abrasive-blasting operations [as required by 1910.94(a)(5)(iv)]. b. On or about February 17, 2026, the employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees and implement the necessary written program elements to ensure that employees voluntarily using Miller Electric half-face respirators with P100 filters and 3M N95 disposable particulate respirators were medically able to use them safely.

29 CFR 1910.252(b)(2)(iii): Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles. a. On or about February 17, 2026, in the welding department, welders performed MIG welding on heat exchanger equipment with only partial barriers in place. The welding curtains did not block the line of sight to the arc, thereby exposing employees working and walking nearby to be exposed to arc?flash hazards.
