Carolina:  The employer did not furnish each of his employees conditions
of employment and
a place of employment free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees in that employees were exposed to a
table saw blade that
could cause lacerations to hands and/or fingers.
a)     worksite-A table circular handfed ripsaw was not guarded by an
automatically adjusting
hood which completely enclosed that portion of the saw above the table and
above the material
being cut.
a)Maintenance Shop-The saw blade was exposed because of the broken guard
that
needed replacing.nce

employees were exposed to a pedestal grinder that was not equipped
with guards.
a)     worksite-A Skil pedestal grinder used for off hand grinding
was not guarded against contact by employees.
a)Maintenance Shop-The Skil pedestal grinder with pieced
wheels on both arbors was missing all guards from the
left arbor and the tool rest from the right side.

rigid
metallic finished electrical cord that was listed and labeled for permanent
installation, the plug was listed and labeled to be attached to a flexible
cord and
not to be used as a pendant on permanent wiring.nt
