Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to severe burns from the potential dust explosion,
deflagration and
fire hazards caused by the improper design and maintenance of dust
collection systems:
(a)Tire retreading area - The following deficiencies were found with dust
collection systems:
(1)  All components of the pneumatic conveying system were not dust tight.
(2)  Area(s) where combustible dust was produced and processed was not
detached,
segregated or separated from other occupancies.
(3)  Isolation devices were not provided to prevent deflagration
propagation between
pieces of equipment connected by ductwork.
(4)  Isolation devices were not provided to prevent deflagration
propagation from air-
material separators upstream to work areas.
(5)  Branch lines of the pneumatic conveying system were disconnected and
used
portions of the system were blanked off without ensuring balanced airflow.
(6)  Explosion prevention or isolation systems were not installed to
prevent the
propagation of flame from the fan to other equipment.
(7)  Foreign material, such as tramp metal, were not removed from the
process
stream.
(8)  All dust collection system components were not conductive.  Rubber,
PVC,
plastic, duct tape and plastic wrap were used to connect sections of the
ductwork to each
other, the cyclone and fan system.
(9)  The dust collection system was not bonded and grounded throughout.
(10)  Inspection, testing and maintenance programs were not implemented on
the dust
collection system.  This includes, but is not limited to, ductwork, air
moving devices and air
material separators.
(11)  A housekeeping program that included using compressed air to remove
combustible dust was implemented without first eliminating all ignition
sources.
(12)  Spark detection and intervention system was not installed in the
dust collector
system.
(13)  Deflagration venting was not installed in the dust collection system.
Abatement note:  Among other methods, one feasible and acceptable means of
abatement
would be to comply with the guidelines outlined in the National Fire
Protection Association
(NFPA) 654 "Standard for the Prevention of Fire and Dust Explosions from
the
Manufacturing, Processing, and Handling of Combustible Particulate Solids"
2007 Edition;
NFPA
68 "Standard on Explosion Protection by Deflagration Venting" 2007
Edition; and NFPA
"Standard on Explosion Prevention Systems" 2002 Edition.
NOTE:  29 CFR 1903.19 REQUIRES THAT YOU PROVIDE CERTIFICATION AND
DOCUMENTATION OF ABATEMENT FOR THIS ITEM.
