29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean condition. Retread Shop, Buffer Area: On, or about, January 29, 2026, material that had been buffed off tires (buffings) had accumulated on the floor and other horizontal work surfaces in the work area. A lab analysis determined that the material was combustible.

29 CFR 1910.303(b)(7)(iii): Internal parts of electrical equipment, including busbars, wiring terminals, insulators, and other surfaces, were damaged or contaminated by foreign materials such as paint, plaster, cleaners, abrasives, or corrosive residues: Retread Shop, Final Inspection Area: On, or about, January 29, 2026, dust and debris generated during the retreading operation was observed on upward facing electrical outlets. The orientation of the outlets would allow dust and debris to migrate inside of the receptacles.
