A FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS TO
FOLLOW THE PROCEDURES OUTLINED IN ASME B30.4-2003 INSPECTION TESTING
AND MAINTENANCE SECTION 4-2.2.3. WHICH STATES DATED REPORTS AND
RECORDS SHALL BE MADE AND AVAILABLE FOR ALL TESTS OF REPAIRED,
ALTERED OR MODIFIED CRANES REQUIRED UNDER PARAS. 4-2.2.1 AND 4-2.2.2.
ANOTHER FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD
WOULD BE TO ESTABLISH A CRANE MAINTENANCE PROGRAM AS REQUIRED IN
ASME B30.4-2003 SECTION 4-2.3.1, WHICH STATES A PREVENTATIVE
MAINTENANCE PROGRAM BASED ON THE CRANE MANUFACTURER'S (HYDRALIFT
AMCLYDE) RECOMMENDATIONS SHOULD BE ESTABLISHED.

crashing onto the

a)Floating dry-dock number two, east wingwall: During a docking operation
the dry- dock
was in a nine foot trim to the north and an American Model 203 Revolver
Crane #34
with operator onboard was not secured by the pelican hook provided at the
south end of
the dry-dock or by rail clamps installed on the equipment. A travel brake
failure
along
with the trim on the dock resulted in the crane unintentionally traveling
the length of the
wingwall (850') eventually falling approximately 60' on to the drydock
floor .
b)Floating dry-dock number two, west wingwall: While the dock was in a
nine foot trim
to the north, an American Model 203 Revolver crane #33 was not secured by
the pelican
hook provided at the south end of the dry-dock or by the rail clamps
installed on the
equipment.
ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATING THE HAZARD IS TO
FOLLOW THE PROCEDURES CONTAINED IN U.S. NAVY FLOATING DRY-DOCK
ARCO CHECKLIST FOR BALLASTING OR DEBALLASTING EVOLUTIONS FCR
ENCLOSURE (6) PAGE 6-13B-5 STEP R WHICH REQUIRES THAT PORT AND
STARBOARD CRANES BE CLAMPED DOWN FOR DOCKING.
ANOTHER FEASIBLE AND ACCEPTABLE METHOD WOULD BE TO FOLLOW THE
PROCEDURE OUTLINED IN SAN FRANCISCO DRYDOCK, INC. STANDARD
OPERATING PROCEDURE (SOP) FOR CONFIGURATION CONTROL OF FLOATING
DRYDOCK NUMBER 2 ENCLOSURE A, PARAGRAPH 3,B,5 VESSEL ON DRYDOCK
WHICH STATES BOTH WINGWALL CRANES WILL BE SECURED WHILE BALLASTING
OR DEBALLASTING WITH A NAVY VESSEL ON DOCK.

A FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS TO
FOLLOW THE PROCEDURES OUTLINED IN THE PORTAL, TOWER, AND PEDESTAL
CRANES STANDARD ASME B30.4-2003,MAINTENANCE,  SECTION 4-2.3.1 (b) WHICH
STATES REPLACEMENT PARTS SHALL BE AT LEAST EQUAL TO THE
MANUFACTURER'S SPECIFICATION.

LIFTING SYSTEMS, SECTION 23-3.2.1(b) WHICH  REQUIRES THE PERSONNEL LIFT
AUTHORIZER TO VERIFY THAT THERE ARE NO LESS HAZARDOUS ALTERNATIVES
TO PERFORMING THE WORK, OR PROVIDING ACCESS TO THE AREA. THE
PERSONNEL LIFT SHALL NOT BE AUTHORIZED WHEN LESS HAZARDOUS MEANS
ARE FEASIBLE.
EXAMPLES OF LESS HAZARDOUS MEANS WHICH WERE AVAILABLE TO GAIN
ACCESS TO THE  BARGES WOULD BE TO BOARD THE EMPLOYEES PIERSIDE BY
USE OF GANGWAY OR BY TRANSFERRING FROM A SMALL BOAT INSIDE THE DRY-
DOCK.

STATES "PERFORM A TRIAL LIFT PRIOR TO LIFTING PERSONNEL WITH THE
PLATFORM ON EACH SHIFT AND AFTER ANY CHANGE OF SETUP, LOCATION,
HOIST EQUIPMENT CONFIGURATION OR OPERATORS."
ANOTHER FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD IS
TO FOLLOW THE PROCEDURES OUTLINED IN UNITED STATES CRANES, INC.
SUPER CAGE USER/ OPERATOR GUIDELINES PAGE 16 ITEM (ii)(D), WHICH STATES
THAT A COMPLETE TRIAL RUN SHALL BE PERFORMED TO TEST THE ROUTE OF

BE MADE AND KEPT BY THE PLATFORM USER FOR THE DURATION OF THE
PERSONNEL LIFT OPERATION." FOR PERIODIC INSPECTIONS, DATED RECORDS
FOR THE PLATFORM SHALL BE MADE BY THE PLATFORM OWNER. THE LAST
PERIODIC INSPECTION SHALL BE KEPT WITH THE PLATFORM AND AVAILABLE
FOR REVIEW."
