29 CFR 1910.178(l)(3)(ii)(I):     Powered industrial truck operators did not receive initial training on other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation:        Dry dock for the Golden Princess:  On or about May 12, 2012, and at times prior thereto, an employee was not trained to use a forklift in conjunction with a wire rope to remove a wood block from under a ship's hull.

29 CFR 1915.112(c)(1)(i):  Employer did not ensure chain and chain slings have permanently affixed and legible identification markings as prescribed by the manufacturer indicating the recommended safe working load for the type(s) of hitch(es) used:        Dry Dock for the Golden Princess: On May 12, 2012, and at times prior thereto, a chain secured to a forklift was used to pull a dry dock block from under the cruise ship. The chain did not have identification markings permanently affixed.
