Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Permian Basin Well Service, Rig 102, Jordan - San Andres Unit A-6, Crane County. For the period of time up to and including 03/24/08, the employer failed to provide procedures to employees required to work on / around pumping units conducting jobs such as removing and reseating the horse head. Employees reseating the horse head were not aware they had not gotten the horse head in the proper position, allowing it to fall from the walking beam when weight was applied, exposing employees to struck-by hazards. Among other methods, feasible and acceptable abatement methods to correct the hazards would be: 1. Obtain a copy of the Manufacturer's Installation Manual for the type of pumping unit prior to employees being assigned work on the unit to ensure there are no unusual procedures recommended by the Manufacturer. 2. Implement a written procedure to instruct employees of acceptable company methods to conduct activities involving the removing or reseating of a horse head. 3. Provide all employees training addressing the hazards they may anticipate in the oil field when removing or reseating a horse head prior to employees engaging in this type of oil field activity.
