Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were likely to cause death or serious physical harm to the employees
in that employees
were exposed to struck by/caught between hazards from roof truss collapse
during
installation:
a)3775 North Eagle Road: On April 16, 2008, and times prior thereto,
approximately 11 to 14 trusses, with spans between 45 to 50 feet, were
installed without top cord diagonal bracing.
b)3775 North Eagle Road: On April 16, 2008, and times prior thereto, 45
to 50 foot span trusses were installed without sufficient lateral bracing.
Single rows of metal truss spacers, used as short member lateral
restraints,  were installed one per side, near the midpoint of the top
chords.  Lateral bracing was not installed near the eaves/bearers nor at
the peak.
NOTE: Feasible and acceptable methods of abatement to correct this hazard
would be to
follow either the manufacturer's recommendations on roof truss erection or
the BSCI - B1,
Summary Sheet - Guide for Handling, Installing and Bracing of Metal Plate
Connected Wood
Trusses, and BCSI - B2, Truss Installation & Temporary Restraint/Bracing,
issued by the
Wood Truss Council of America and Truss Plate Institute.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were likely to cause death or serious physical harm to the employees
in that employees
were exposed to struck by/caught between hazards from roof truss damage
and failure:
a)3775 North Eagle Road: On April 16, 2008, the employer used a single
pick point to hoist and install 50 foot roof trusses.
NOTE: Feasible and acceptable methods of abatement to correct this hazard
would be to
follow the manufacturer's recommendations on roof truss erection and the
2006 edition of
BSCI -B1 Summary Sheet - Guide for Handling, Installing, Restraining &
Bracing of
Trusses, issued by the Wood Truss Council of America and Truss Plate
Institute.  This
clearly states "single trusses between 30 feet and 60 feet use a spreader
bar one half to two
thirds of the truss length.  Attach truss to the spreader bar with lines
that slope inward or
"toe-in.  Lines that "toe-out" can cause the truss to buckle".
