Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazard that
were causing or likely to cause death or serious physical harm to
employees, in that employees
were exposed to flying objects hazards:
On or about 09/08/2009 an operator of a horizontal metal lathe was struck
by an unsecured piece
of pipe, causing a fatal injury.
Among other methods one feasible and acceptable abatement method to
correct the hazard is to
relocate any storage or hanging boards from behind the machine operating
switch and to change
the existing switch mechanisms to bush buttons mechanism.
Specific abatement documentation is required.
