Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught by hazards: a.On or about March 9, 2009, an employee operating a Ridgid K-7500 Drain Cleaning Machine, serial #VAK02926F97, to clean out a plugged sewer drain, was injured when the glove they were wearing became caught-by the rotating cable. The air foot switch pedal designed by the manufacturer to allow the operator to deenergize the machine from the operator's position had been removed and the switch it controlled had been bypassed. b.At the facility the Cobra LX1000 Cable Drum Machine's foot switch actuator designed by the manufacturer to allow the operator to deenergize the machine from the operator's position had been removed and the switch it controlled had been bypassed exposing employees to a caught-by hazard. ABATEMENT NOTE: Among other methods, feasible and acceptable abatement methods to correct this hazard are to reinstall the missing Ridgid K-7500 air foot switch pedal and switch and to reinstall the missing Cobra LX1000 foot switch actuator and switch to allow the operator to deenergize the machines from the operating position and to provide training on the safe operation of the machines.
