Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:05/28/08 Location:South Side of Building Condition:A Genie GS 2668 RT scissors lift was used to lift an 18 foot, 4.5 inch long header beam weighing 448.35 lbs, approximately 17 foot, 10 inches into the air for installation. (b)Date of Violation:05/28/08 Location:South Side of Building Condition:The employer lifted a header beam, 18 feet, 4.5 inches long, with a Genie GS 2668 RT scissors lift that had a working platform that measured 5 feet 1 inch wide and 8 feet 3 inches long. The platform when extended was 13 feet long which allowed between 2.8542 feet to 5.125 feet of the header hanging over the railings of the lift. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use a crane to lift and support objects being installed at height above ground and require that appropriate work practices be followed in accordance with the Genie Operator's Manual with Maintenance Information for the GS 2668 RT, Third Edition, Pages 4 and 21. The load must be secured/supported until it is properly installed. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
