not maintained in
accordance with manufacturer specifications and/or industry standards:
a) Jefferson County, Warsaw Township, Sprague Wingard 10 well - On or about
December 22, 2008, and at times prior thereto --- Employees were exposed
to struck-by,
crushing, and blunt force trauma hazards from the Walker-Neer Service Rig
#4 that was
operating with a Bridon American Constructex 6x40 7/8" load line that was
beyond
serviceable limits according to manufacturer specifications and industry
standards as
evidenced by improper lubrication, lack of daily visual inspection, and
omission of a ton-
mile calculation. Specifically, these conditions included:
1. Field lubrication of the load line consisted of used motor oil. This
type of
improper wire rope lubrication contained no corrosion prohibitor and
diminished
the load line service life.
2. Daily visual inspections were not completed to identify broken strands
or rope
defects.
3. A Slipping and Cut-Off preventative maintenance program was not
established.
Service life calculations from Ton-Mile limits were not performed.
ABATEMENT NOTE:
Among others, one feasible and useful abatement method to correct this
hazard is to:
(1) Follow the field lubrication requirements of the Constructex wire rope
manufacturer Bridon American and American Petroleum Institute (API) RP9B
Recommended Practice on Application, Care, and Use of Wire Rope for
Oilfield
Services Paragraph 3.3.8 which states "Lubricate the rope with a good
grade of
lubricant which will penetrate and adhere to the rope."
(2) Reference the wire rope inspection criteria from Bridon American and
American Petroleum Institute (API) RP54 Recommended Practice for
Occupational Safety for Oil and Gas Well Drilling and Servicing Operations
Paragraph 9.6.1 which states "All hoist lines be visually inspected once a
day."
(3) Reference the cut off and rope maintenance program from Bridon American
and American Petroleum Institute (API) RP9B Recommended Practice on
Application, Care, and Use of Wire Rope for Oilfield Services Section 6
Slipping
and Cutoff Practice

Specifically, these conditions existed on the following ground anchor
components:
1. Rear Ground Anchors - The two (2) rear ground anchors had not been set.
2. Forward Ground Anchors - The two (2) forward ground anchors were
positioned at improper angles and distances from the service rig.
3. Ground Anchors - The steel stakes were not installed according to any
written
instructions or pull tested to determine load capacity
ABATEMENT NOTE:
Among others, one feasible and useful abatement method to correct this
hazard is to:
(1) Follow the requirements of Walker-Neer Manufacturer Co., Inc.
specification
sheet which requires four (4) ground anchors connected to crown guy wires.
(2) Reference American Petroleum Institute (API) RP4G Recommended Practice
for Use and Procedures for Inspection, Maintenance, and Repair of Drilling
and
Well Servicing Structures, Section 14 Guy Wire Anchors for Portable Masts
with
Guy Lines
(3) Reference American Petroleum Institute (API) RP54 Recommended Practice
for Occupational Safety for Oil and Gas Well Drilling and Servicing
Operations,
Paragraph 6.3.2 which states that "Well operations involving the rig shall
not be
commenced until the rig is rigged up in a safe manner."

guy wire

1. Marking of Guy Wires - Visible markers were not placed on each guy wire
to
prevent accidental contact and resulting physical damage from heavy
equipment
and vehicle traffic.
2. Come-Along Rigging Equipment - A "chain-around" system did not bypass
the
come-alongs that were used to tension the guy wires. The come-alongs were
designed to tension the guy wires and were not designed/installed to
transmit the
load from the guy wires to the anchors.
ABATEMENT NOTE:
Among others, one feasible and useful abatement method to correct this
hazard is to:
(1) Follow the requirements of Walker-Neer Manufacturer Co., Inc.
specification
sheet which requires four (4) crown guy wires from the mast be connected to
ground anchors.
(2) Reference American Petroleum Institute (API) RP54 Recommended Practice
for Occupational Safety for Oil and Gas Well Drilling and Servicing
Operations,
Paragraph 6.3.2 which states that "Well operations involving the rig shall
not be

anticipated workplace fires and the size and degree of hazard which would
affect their use:

December 22, 2008, the employer did not provide a minimum of two (2) 20
pound
capacity fire extinguisher with a Class BC rating. The Walker-Neer C33A
service rig had
only one (1) 20 pound ABC fire extinguisher mounted on the side rack.
ABATEMENT NOTE:
Among other methods, feasible and acceptable means to correct the
condition include:
1. Complying with the applicable requirements referenced in American
Petroleum
Institute (API) RP 54, Section 7.2.5 Fire Protection
2. Procuring additional fire extinguisher and deploying them at the site.
3. Develop and implement a comprehensive fire protection program that
analyze fire
hazards and ensures proper fire protection equipment is deployed at the
site to adequately
deal with the fire hazards.
