Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an open flame heater operated near the rig floor: a) Kleinert 1, Spring Township, Crawford County, PA - On or about October 9, 2009, on PEP Drilling Co. Rig #10, employees were exposed to a fire hazard from an unapproved heater inside the dog house trailer. A homemade stove fabricated from a single piece of steel pipe was identified as an ignition source in close proximity to the well bore. Condition created a potential fire/expolsion hazard. ABATEMENT NOTE: Among others, one feasible and useful abatement method to correct this hazard is to: (1) Follow the requirements Reference American Petroleum Institute (API) recommended Practice RP 54, Paragraph 7.1.8, page 14ge

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to working from an elevated work platform without properly installed emergency escape line and Geronimo device: a)Kleinert 1, Spring Township, Crawford County, PA - On or about October 9, 2009, on PEP Drilling Co. Rig #10, the Geronimo line was not installed for employees to use in case of an emergency evacuation from the derrick board. The derrick board was located sixty-five feet (65') above the ground. Rig #10, the goose neck pipe for the drilling mud line as it connects to the flexible hose was not secured with a safety straps to prevent the hose from falling in the event of failure under pressure. Condition created potential struck-by hazards. ABATEMENT REFERENCE: Among others, one feasible and useful abatement method to correct this hazard
