Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to struck by hazards associated to screwpile pipe
loading operations:
(a) Alpine Site Services, Inc., 6869 Holly St., Commerce City, CO 80022:
The
employer did not ensure that screwpile loading operations were free from
recognized
hazards in that a screwpile pipe fell during loading operations. The
employer did not
ensure that the screwpile pipes loaded by forklift on a flatbed truck were
safely
secured to prevent them from rolling off. On 4/8/08, a pipe rolled off and
struck an
employee on the back, which resulted in the need for medical attention.
This
condition exposed employees to hazards including, but not limited to,
death,
broken
bones, and contusions.
Among other methods, one feasible and acceptable method to correct this is
hazards is the
use of engineered controls and/or devices to prevent the screwpile piping
from rolling such
as:
(1) install vertical members such as stakes, pegs, side boards, or pins in
dunnage;
(2) placement of devices such as wedges, safety blocks, or chocks;
(3) modify dunnage to include raised edges at the ends;
(4) minimum use of two employees on the ground handling both ends of the
pipe;
(5) develop and implement pipe loading procedures accompanied with formal
training.

enclosed "Sample
Abatement-Certification Letter").
