Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) DHS Drilling Company, Vega 33-441, Collbran, CO: The employer had not ensured that prior to drilling into formations that may contain unexpected gas pressures, the flexible 5000 pound rated choke hose between the manifold and Blowout Preventer (BOP) was secured with safety clamps to minimize "whipping" in the event of hose rupture. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the "corrective actions" section listed in the "Safety Alert" (#00-24 & #98-23) bulletins published by the International Association of Drilling Contractors (IADC) thereby ensuring that properly rated, placed, and secured safety clamps are utilized on all high pressure hose. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) DHS Drilling Company, Vega 33-441, Collbran, CO: On or about July 9, 2008, the employer had not ensured that the path of travel on the auxiliary escape line was without obstacles. A vehicle and a 55 gallon barrel of material was placed within the path of travel. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that all escape line routes are kept clear of obstructions as referenced by the American Petroleum Institute, API RP 54 1999 in section 6.10.1 Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
