General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: On or about September 5, 2008 at the oil and gas site located at Mercer Well #78, County Road 2331, Johnson County, Texas: Employee working a workover rig was exposed to fire and explosion hazards in that the auxiliary escape line was not set up for the employee to quickly evacuate the derrick platform in the event of an emergency. Among other methods, one feasible and acceptable method to correct this hazard is to install the auxiliary escape line route. The employer should use a specially rigged and securely anchored escape line attached to the derrick or mast so as to provide a ready and convenient means of escape for the derrick man's working platform as referred in paragraph 6.10.1 of AP Recommended Practice for Occupational Safety for OIl and Gas Well Drilling and Servicing Operations, 1999 edition.
