Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which was free from
recognized hazards
which were likely to cause death or serious physical harm to employees, in
that employees
were exposed to struck by hazards:
a) On or about March 19, 2009 and at times prior, employees were exposed
to injury while
operating Rig #9629 in that, the clamps on the guy lines were spaced 9 1/2
inches apart.
Among other methods, one feasible and acceptable method to correct this
hazard condition
would be to install the clamps in accordance with Table 2 of Section 3.7.4
of the American
Petroleum Institute Recommended Practice 9B on Application, Care and Use
of Wire Rope
for Oil Field Service.
