recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that,
employees were exposed to explosion and struck-by hazards:
a) Chesapeake Energy, Mowry 1H well site, Clapper Hill Rd., Laceyville,
PA: Two
employees were injured while trying to escape from the snubber floor
during a
pressure release from the well bore, on or about 01/28/10.
Among other methods, one feasible and acceptable method to abate this
hazard is to:
1) Ensure an individual escape line is rigged and available for each
person working atop
hydraulic snubbing equipment in accordance with American Petroleum
Institute API RP-54,
Recommended Practice for Occupational Safety of Oil and Gas Well Drilling
and
Servicing
Operations Manual, page 27, section 14.2.2.
