Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to struck-by and crushed-by hazards: At the worksite: On or about July 18, 2008, and at times prior thereto, the employer required employees to use a well service rig (workover rig) without the drill line retaining device properly installed. Employees were exposed to the type of hazards associated with struck-by due to failure of the drill line fastening device. Among others, one feasible and acceptable method of abatement would be to comply with American Petroleum Institute (API) Recommended Practice 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, generally accepted industry practice and manufacturer requirements.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to falls and struck-by hazards associated with structural collapse: At the worksite: On or about July 18, 2008, and at times prior thereto, the employer required employees to use a well service rig (workover rig) without the rod basket guylines connected and tensioned to anchor points. Employees were exposed to the type of hazards associated with failure of the well service rig derrick due to improper guyline support during rig operations. Among others, one feasible and acceptable method of abatement would be to comply with American Petroleum Institute (API) Recommended Practice 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, generally accepted industry practice(s) and manufacturer requirements.

COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THE OCCUPATIONAL SAFETY AND HEALTH ACT OF 1970, SECTION 5(a)(1), EMPLOYER DID NOT FURNISH EMPLOYMENT AND A PLACE OF EMPLOYMENT WHICH WERE FREE FROM RECOGNIZED HAZARDS THAT WERE CAUSING OR LIKELY TO CAUSE DEATH OR SERIOUS PHYSICAL HARM BY NOT PROVIDING A SECONDARY MEANS OF EGRESS WHILE WORKING IN THE DERRICK OF A WELL SERVICE RIG WAS CONTAINED IN OSHA INSPECTION 309791499 CITATION NUMBER 01, ITEM 001, ISSUED ON 04/26/07, WITH RESPECT TO A WORKPLACE AT 10 MILES EAST, 8 MILES SOUTH, WAYNOKA, OKLAHOMA 73860. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazard of fire, explosion, oil/gas well blowout and fall from elevation: At the worksite: On or about July 18, 2008, and at times prior thereto, the employer required employees to perform work from elevated heights of a well service rig derrick without a secondary means of egress properly installed. Employees were exposed to the type of hazards associated with not being able to egress the derrick in the event of an emergency. Among others, one feasible and acceptable method of abatement would be to comply with American Petroleum Institute (API) Recommended Practice 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, generally accepted industry practice(s) and manufacturer requirements.
