Section 5(a)(1) of the Occupational safety and Health Act of 1970:  the
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees:
Entire facility - On or about April 21, 2009, an employee was allowed to
work at heights greater
than 10 feet from a platform supported by the forks of an industrial truck
in order to retrieve
stored merchandise. The cage was not securely attached to the industrial
truck and thereby the
employee was exposed to a fall in the event the platform fell off the
forks.
Among other methods, feasible and acceptable abatement methods include
following the
requirements for lifting personnel explained in the ANSI/ASME B56.1-2000
Safety Standard for
Low and High Lift Trucks.
