Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to an elevated fall hazard from an order selector stand up
lift truck:
(a)Warehouse - employees who operate the Clark stand up Type E electric
order selector
lift truck, Serial Number OP1S0129-PM9293, Model Number OP1S, Yale stand
up Type E
electric order selector lift truck, Serial Number B801N01856W, Model Number
OS030EBN24TE089 and Yale stand up Type E electric order selector lift
truck, Serial Number
N594507, Model; Number OSO3QEAN24TE089 were not provided with fall
protection.
Employees were exposed to an elevated fall hazard of 4 foot to 12 foot
high from the ground
level while standing on the order picker to retrieve small boxes of
materials
Among other methods, one feasible and acceptable method to correct this
hazard includes but
not limited to the following:
In accordance with ANSI B56.1 2005 - American National Standard Institute
for Safety Standard
for Low Lift and High Lift Trucks; Section 4.17.2 (c) "Whenever a truck is
used to elevate
personnel, the following precaution for protection of personnel shall be
taken in that be certain
that required restraining means such as railings, chains, cable, body
belt(s) with lanyard(s) or
deceleration devices, etc are in place and properly used and Section
7.37.1 "Platforms used for
elevating personnel shall have restraining means such as a guard rail or a
means for securing
personnel such as a body belt or lanyard whenever the platform can be
elevated to a high greater
than 1200 mm (47.2 inches).
No abatement certification or documentation is required for this item.
