(a) On August 16, 2022, the employer did not ensure that ladders were inspected before initial use on each work shift, and more frequently as necessary, to identify any visible defects.  An 8-foot A-frame ladder was used on August 16, 2022 to retrieve fittings from an elevated storage racks. There were visible defects on the ladder including 1 bent rung, 1 missing rear horizontal support,  4 bent rear horizontal supports, and 2 bent spreaders.
